- View your provider’s notes from you previous appointments
- Send a direct message to your provider
- Log in from any web browser, including mobile

New User?
If you are a current patient but have not set up an account in our Electronic Medical Record portal, you may either call your provider’s office to request a new account registration link be emailed to you, or email your provider’s office and request access. Please include the following information in your request to access your electronic medical record:
